This consent form can be used to obtain consent from parents on the use of online platforms till the end of this school year. This information will be shared only through a secure FTP site (file transfer protocol) (similar to online banking) and will only be used for the purpose of student fee management and online payments. All payment information is stored on a secure site within Canada.
The only way a parent/guardian can access a student's account information in School Cash Online will be by entering the following information:
- school student attending
- student legal last name
- student PEN number (see Student Report Card)
- student date of birth

Throughout the school year, within the school day as per Teacher/School planning, your child may be included in:

Please Note: Teachers will send home essential details of planned field trips and excursions.
Special parent/guardian permission will be sought for non-routine trips out of the local area.

Dear Parent(s) and Guardians,
School District #62 (Sooke) will be allowing students in Kindergarten through grade 12 access to a district-managed Google Apps for Education (GAFE) account. This account provides students with a powerful collection of online collaboration and productivity tools, as well as online file storage to be used for educational purposes.
Each staff and student will have their own secure login and password and receive instruction on how to appropriately use the GAFE platform in ways that protect personal student information.
Student account information (student first, last name and grade level), as well as any documents uploaded onto the Google Apps platform will be disclosed to Google who hosts and stores this information on secure servers located outside of Canada. Inside of the GAFE platform there are no ads, no data-mining, or selling of data. For more info please visit https://www.google.com/edu/trust/. This information is still subject to the laws of foreign jurisdictions including, the USA Patriot Act. Privacy legislation requires that we inform you of this and obtain your consent to this arrangement.

I have read "Learning Resources" (Regulation B-115). I have also signed the school-provided form responsible technology use and returned it to the school. I understand that my child's information will be used for Google Apps for Education.
This consent will be considered valid from the date at which it is signed until which point the student named above is no longer a student within the school district or permission has been withdrawn.

Parental/Guardian Consent for Internet Access
Legal and ethical considerations make it imperative that all students who access the Internet from a school-sponsored account understand and agree to abide by the District's Internet Use Agreement, as detailed below. Please go over the agreement with your son or daughter to be sure that you both understand it. If you wish your child to have an Internet Access account please provide your consent below.
Introduction
The interconnected world-wide web of computers known as the Internet can provide students with access to the most recent research and the most up-to-date statistics and opinions. Families should be aware that some material accessible via the Internet may contain items that are illegal, inaccurate or potentially offensive to some people. While our intent is to make the Internet access available to further educational goals and objectives, students may find ways to access inappropriate materials as well. It is, in fact impossible to completely protect students from accidental exposure to inappropriate materials. For this reason it is important that the District have an acceptable use agreement providing guidelines for the use of this vital information resource.
Acceptable Uses of School-Sponsored Internet Accounts:
- Research for school assignments
- Personal research on appropriate topics
- Communication through email for school assignments
- Appropriate personal communication through email
Unacceptable Uses of School-Sponsored Internet Account:
- Uploading to the Internet material that is copyrighted, obscene, offensive or defamatory, that advocates or condones illegal activity, or that contains a computer virus.
- Downloading from the Internet material that is copyrighted, obscene, offensive or defamatory, that advocates or condones illegal activity, or that contains a computer virus.
- Engaging in any unethical behavior, including but not limited to invasion of privacy. This would include gaining unauthorized access to any computer system, record or correspondence.
- Endangering any person's safety by publicizing their phone number, address or other personal information without that person's explicit written approval.
- Monopolizing time and bandwidth by accessing the Internet from a school account to conduct business, play games or use chat lines or to send or receive a large number or personal messages.
Consequences of Unacceptable Use may include but are not limited to:
- Suspension of Internet access privileges
- Other disciplinary action determined by the school administration
- Involvement of law enforcement agencies when applicable
I understand the School District's Internet Use Agreement as stated above. I give permission for my child to receive and make use of a school sponsored Internet access account.

In accordance with the BC Freedom of Information and Protection of Privacy Act, the Sooke School District is seeking your consent to collect, retain, use and disclose photographs, videos, images and/or names of students and groups of students in a variety of publications and on the School District's website(s) for educational purposes, such as recognizing and encouraging student achievement and for the purposes of building the school community and informing others about the school district, its programs and activities.

The Sooke School District is using the MyBlueprint platform for career education, digital portfolios, and self-assessment of core competencies throughout kindergarten to grade 12.
MyBlueprint is a user-friendly, Canadian-based platform offering curriculum related to career education and storage space for digital portfolios for learning, communicating progress and assessment. The service supports teachers, empowers students, and engages parents. MyBlueprint increases engagement by offering a continuous glimpse into each child's learning that teachers, parents and students share.
Accessible via website on any browser, MyBlueprint captures current progress in the classroom and offers a more complete representation of a child's learning than a report card. Teachers set up classrooms with individual student and parent accounts to manage, view, and comment on demonstrations of learning in the student's digital portfolio.
Teachers will be explicit with student expectations and acceptable uses of MyBlueprint for learning. Teachers will moderate what and how information may be uploaded. Student information is accessible only by the student, teacher, and parent for educational purposes. Your consent to your child's use of MyBlueprint is required by British Columbia's Freedom of Information and Protection of Privacy Act (FIPPA).

Your child's teacher is planning to use an on-line tool called SpacesEDU as part of communicating student learning for your child this year. You can visit their website at https://spacesedu.com/en/
The specific educational purposes for using this online tool includes creating electronic portfolios, making this information easily available to parents/guardians, the student themselves, selected teachers, and the principal.
In order to use of this tool, the following specific information about your child will be entered into SpacesEDU:
- student name;
- student e-mail address;
- all content that the student creates or shares in video, text, images, and other media files that they provide on or through the Services, or that are provided on their behalf;
When students use our Services, we will also know what school they attend and the city and state or province in which they go to school, and we may combine that information with information we have from their account.
Privacy legislation requires that we inform you of the location where your child's data will be stored prior to obtaining your consent to use this tool. As such, according to the terms of service, the information about your child will be stored within Canada as part of the service's normal operations. SpacesEDU maintain technical, physical and administrative security safeguards to protect your personal information against loss, theft, and unauthorized access. Any personal information that you provide is exchanged on a secure server. We store data in a highly secure Microsoft Azure datacenter. Microsoft Azure provides industry-leading compliance and privacy certifications to keep student data in Spaces safe. The consent is considered valid from the date at which it is signed until the following condition applies (to be monitored by the teacher requesting consent): The student named below is no longer registered within the school district, or permission has been withdrawn.
Upon the expiry or withdrawal of this consent, SpacesEDU will receive a request to remove the student data from their system. This consent allows your child to use specific web-based applications for the purposes of instruction and learning.
The district implements a comprehensive and ongoing approval process of web-based applications. This process ensures that applications approved by the district meet the relevant district and provincial standards of privacy. The review process includes evaluating how data is collected, used, and disclosed to ensure personal information is protected. The district also ensures that the use of the application meets the requirements set out under the BC Freedom of Information and Privacy Act (FOIPPA) before it is approved for use. For more information on how your child's personal information and privacy is protected please review the Student Privacy FAQ document found on the District website.
Your consent is required before your child can use these applications. If you have questions about how or why a specific web-based application is being used with your child, you are encouraged to contact your child's teacher.
The use of these applications is not an educational requirement for students. Students are not obligated to use these applications and all activities that rely on the use of these applications will have some equivalent means of student participation.
To facilitate the collection of consent, the district is requesting a single consent from you that will allow your child to use any of these district-approved applications.
